OFFICIAL RULES
1. HOW TO ENTER: NO PURCHASE NECESSARY TO ENTER OR WIN; A PURCHASE WILL NOT IMPROVE CHANCES OF WINNING
In order to enter, from Friday, June 13, 2014, through 11:59:59 p.m. PST on Wednesday, June 18, 2014, go to the www.Warm1069.com website and complete the entry form, which includes your name, email address, address and telephone number, and submit a photograph of your Dad or yourself if you are a Dad; the Dad whose picture you submit must be at least 18 years of age or older. Photo collage submissions are not allowed; only single photos will be accepted. You must have the permission of the Dad whose photograph you are submitting to enter him in this sweepstakes. WARM 106.9 is not responsible for problems accessing the website or completing the entry, technical or atmospheric conditions that disrupt the completion of the entry, or any other problems which might or might not result from this contest. Any person submitting more than one entry or an entry without a photograph attached may be disqualified at WARM 106.9's sole discretion.
2. HOW TO PLAY/HOW THE WINNER IS CHOSEN: Enter as explained in Paragraph 1 above. All entries will be posted on www.Warm1069.com from 12:00:01 a.m. PST on Thursday, June 19, 2014, through 11:59:59 p.m. PST on Sunday, June 22, 2014 ("public voting period") to determine the winner; during this public voting period listeners can vote for their favorite entry(ies); no limit on the number of times a listener can vote during the public voting period. The entry with the highest number of votes will be the first place winner, and the entry who receives the second most number of votes will be the second place winner. The winners will be notified by telephone and/or email. Limit: one (1) winner per household. In the event of a tie, winner(s) will be selected by random drawing from among all tying entries.
3. CLAIMING A PRIZE: The winners must complete the required paperwork within thirty (30) business days of the day he or she wins to verify that he/she is a qualified winner. Failure to comply with this deadline shall be deemed to be the winner’s forfeiture of his or her right to claim the prize. The winner must prove he or she is a qualified winner by presenting identification acceptable to WARM 106.9 (e.g., state drivers license; state issued identification card; valid passport with address verification; or birth certificate with social security card and photo identification and address verification). If a winner cannot prove he or she is a qualified winner, the prize will not be awarded. All unclaimed prizes remain the property of WARM 106.9. Prizes will not be issued to winners unless a social security number is presented. Once the required paperwork has been completed, a winner must pick up his/her prize at the WARM 106.9 Studios, 3650 131st Avenue S.E., Suite 550, Bellevue, Washington, within thirty (30) days of the day he or she wins the prize.
4. PRIZE: The prize will be awarded to the person who entered this sweepstakes.
a. First place winner chosen pursuant to Paragraph 2 above. There will be one (1) first place winner. The winner will receive the following:
i. An iPad Air, Model No. MD788LL/A, Wi-Fi, 16GB Silver; value: $499. Any warranties and guarantees are strictly limited to the manufacturer’s.
ii. One (1) six month subscription to "Next Issue." Valid for six (6) months from date of redemption of the prize.
iii. $200 in American Express gift cards; the cards expire on December 31, 2014. The cards are subject to the policies, practices and procedures governing American Express gift cards.
b. Second place winner chosen pursuant to Paragraph 2 above. There will be one (1) second place winner. The winner will receive $200 in American Express gift cards; the cards expire on December 31, 2014. The cards are subject to the policies, practices and procedures governing American Express gift cards.
A qualified winner must also meet the eligibility requirements as set forth in Paragraph 5 of these rules. Each winner is responsible for all applicable income taxes. All federal, state and local laws apply. A 1099 form may be submitted to all appropriate taxing authorities.
5. ELIGIBILITY: Individuals must be 18 years or older and residents of Washington and reside within the WARM 106.9 regular listening/broadcast area to be eligible to enter or win. In order to be eligible to win a prize in this sweepstakes, you cannot have won any prize from WARM 106.9 of any value within 30 days prior to the awarding of a prize to you in this sweepstakes; or a prize or combination of prizes with a total stated value of $500.00 or more from WARM 106.9 within 180 days prior to the awarding of a prize to you in this sweepstakes. These restrictions apply even if you reject the prize. This waiting period of 30 or 180 days does NOT apply to movie premiere events; winning movie premiere event tickets does not count toward any prize waiting period. Employees of Hubbard Radio Washington, LLC, Hubbard Radio, LLC, WARM-FM and Next Issue, or any Seattle, Washington, radio station, television station or media outlet, or their respective parent companies, subsidiaries, affiliates and their immediate families, are not eligible to enter or win. Anyone participating in this sweepstakes or claiming a prize in this sweepstakes in violation of these rules will be disqualified from winning a prize in this sweepstakes and from participating in WARM 106.9 sweepstakes for a period of one year. WARM 106.9may discontinue the sweepstakes at any time without notice. WARM 106.9may also at any time and without notice change the prizes to be awarded in this sweepstakes.
By entering this sweepstakes, each entrant is certifying that he or she is not a professional photographer, that the submitted photograph is his or her own original work, and that he or she has the right to allow the publication of the photograph without restriction, and that WARM 106.9's use of the photograph will not infringe the rights of any third parties (e.g., copyrights, rights of publicity, etc.).
6. RELEASE; CONSENT TO USE LIKENESS, VOICE AND ADDRESS: Each winner may be required to execute an affidavit of eligibility prior to awarding of a prize. Each winner (a) agrees to release Hubbard Radio Washington, LLC, Hubbard Radio, LLC, WARM-FM and Next Issue from any and all liability, claims, damages and actions arising directly or indirectly out of or sustained in connection with the awarding or acceptance of the prize(s), and/or misuse of any prize, or to participation in the sweepstakes or in prize-related activities, including any travel related thereto, and (b) consents to the use by WARM 106.9 for promotional purposes, of his or her likeness, voice (including the posting of the photo entry on WARM's website) and/or address (city/state) without any additional compensation. By participating in this sweepstakes, each participant agrees to the application of these official rules.
7. WINNERS LIST: For the names of the winners, send a self-addressed, stamped envelope before July 15, 2014, to Winners List, WARM 106.9, My Dad Rocks Photo Sweepstakes, 3650 131st Ave SE, Suite #550, Bellevue, WA, 98006.
8. MISCELLANEOUS:
Decisions of WARM 106.9 concerning any and all matters with respect to this sweepstakes shall be final. Sweepstakes void where prohibited or otherwise restricted by law. WARM 106.9 reserves the right to change the prize values at any time. WARM 106.9 is not responsible for printing errors in the sweepstakes materials or these rules.
